Has recently lost its green hue and I no longer see the polyps. Also starting to have white and brown edges. Everything was fine until I glued it to the rock and got a bit of epoxy on the base. Is it dead?

It's dead when all the tissue is gone, you see white skeleton, and algae starts growing on it.
This one is in bad shape for sure and has lost tissue in a few places but could possibly still recover.
The glue probably irritated it but that might not be the whole issue.
In that photo it looks like it may be getting too much light.
Could try moving it down temporarily.

Its not dead yet but way unhappy. You say you just glued it down- did you move it to a much higher lit area? If so dim the lights, it needs to acclimate or move the rock. Did it come from a different tank as well? It may just be in shock, I'd lower it or lower the lights as a first step. Parameters?
