Kole tang looking bad. Help ID

Went through the pictorial guide sticky but really couldn’t walk away with a solid conclusion. Please help ID. Not eating and breathing rapidly. =(

Can you look at the sticky about hints and tips regarding posting a problem report? That will give you some idea as to the background history that we need to have first.
How long has it been like this, what are the water Chemistries, what other fish are in with it, how long have you had it, how was it quarantined, etc. A short video is also helpful. Have you tried any treatments yet?

I see mucus production likely from an external parasite, but I can’t say which one, in descending order; brooklynella, bacteria from an injury (is its mouth and tail fin damaged?), Amyloodinium or even a advanced case of ich.

• 100gal mixed reef tank 6 months old
• I’ve had the Kole tang for 4 months,
• it’s tankmates are lemonpeel Angel, yellow tang, blue hippo tang, pintail fairy wrasse and 2 bimac anthias
• All the fish went through a strict QT from marine collectors (I know it includes many treatments and every fish I’ve ever got from there is extremely healthy. I can get you the exact QT details if needed)
• First noticed today but thinking back didn’t see the Kole eat yesterday either. So two days possibly
• tail fin and mouth do look slightly damaged
• Water is Ammonia 0.00, Nitrite 0.0, Nitrate 5ppm, Phosphate .03ppm, PH 7.9
  Salinity 1.026
• No new fish has been added for 3 months
• Recent changes Added copepods from online vendor, and some macro algae
• Tank had an algae bloom from auto feeder going crazy while out of town. Still dealing with some reminisce but mostly in the clear
• Things to note tangs are aggressive toward each other
• Added snails and shrimp about 2 months ago

Based on the history, I think you can rule out the common protozoan diseases. Flukes are also unlikely. Just spit balling here - what if the tangs go into a fracas and this one ran into some rocks and got wedged in/damaged?
