I'm sure the switch to sodium bicarbonate from calcium hydroxide played a role and maybe you did get an off batch of salt, but the extremely high phosphates (2-3) coupled with a rapid drop in phosphates are what I believe are the root cause.
You've already mostly corrected the problem by bringing po4 down to .1 (smart stopping at .1 by the way).
Be slower about bringing po4 down farther.
GFO can be a lot more dangerous than most realize and some systems don't tolerate it at all.
