Lost Tang during Copper Ich Treatment

So I have been treating Ich on my Yellow Tang, Kole Tang and clowns since Thursday (making today day 5).

I came home from work tonight to find my tang upside down on the bottom motionless ;Dead The yellow tang had shown white spot on the pectoral fins last weekend.

What did I do wrong?

Temp- 28oC - a little high I admit. Have turned temp down now!
PH - 8.1
Amonia - 0
Nitrate - 10
Nitrite - 0
Copper 0.5

I’m using NB labs anti parasite treatment.

I’ve got a powerhead in there pointing at the surface. It’s a 120L tank with 100L in it.

^^ Need more information about this copper product, as I am unfamiliar with it.

Is it chelated or ionic copper? Or possibly copper sulfate?? What test kit are you using?

Label says it’s:
copper sulphate 1600 mg/100ml
Formaldehyde 2040 mg/100ml

I don't know that it's an oxygen issue. But it is recommended to use an Airstone as well. I feel like more is better. Fish are already going through alot of stress with the parasites, and then the heavy meds. So if you can make the breathing part easier the better off you are. Keep in mind the parasites do affect the gills heavily. Good luck, hopefully now that humble has your copper ingredients he will have additional insight as well
 
I wouldn’t personally mix formaldehyde and copper, never heard of that product either!
