LPS Corals are dying

What can cause LPS corals to die off? Starvation?
Are there pests only for LPS?

I've had Blastos then Acans die, then Challices and now even a Scoly is dying.

Zoas are doing great... Acros are doing great, and even my Mushrooms are great... I even have Pulsating Zenias, Rock Nems and Montis as well as Cloves and all are Growing well...

What can cause this!

I dip everything before I introduce it... But I did discover a bristle worm in the Scoly and I took care of it... Not sure if there are others. But I truly am surprised to see it since I dip EVERYTHING before I put it in.

Help me Obi Wan Kenobi...

pH is between 8.0-8.3
Cal: 450
SAL: 34
ALK: 9.4
MAG: 1340

PO4 is low,
NO3 < 10

Tanks was restarted in March 2020. So Corals have been doing great! My worry is maybe not enough food? Although I feed plenty But not sure if its enough now :(

Several things can cause this:
High salinity (especially from false readings- calibrate your tester )
too much light
high phosphates and nitrates
High calcium
Black bugs on or underneath coral
Fish such as butterflies and angels

Pics show many of these on tank bottom which may be getting sand on them and irritating them. Elevate them a little Even on top of a flat rock.

Moderate light and water flow
salinity 1.025
ph 8.1 -8.3
temp 77-79
Phos < .04
nitrate < .04
Ammonia < .03

ADD A BAG OF CHEMIPURE BLUE OR ELITE. IN A MIXED REEF SETTING BOTH LPS AND SPS CAN RELEASE TOXINS AND CAUSE CHEMICAL WARFARE AND TISSUE LOSS

I run Carbon and GFO Reactor, and for Fish I have 1 Mid Size Tangs, 3 Anthias, 3 clowns, 2 Cardinals, 6 Line Wrasse (well behaved), Mandrin, and I had a Copperband which I sumped because it was picking on Acans.

GFO can be an issue or the issue. Ferris oxide is very strong
 
If you had a bristle worms, you will forever and always have them. Make sure your clowns are attempting to host the scoly, I have had that happen, but looks more like a critter is getting a good bite to eat.. I agree with others, get a more conclusive test, so we can pinpoint exactly what is going on!
 
What test kits are you using? That scoly looks like it has been attacked. Do you have any emerald crabs in the tank?
