LPS is failing.plz help

Hi guys .Im in this hobby for 4 years and its the third year that im keeping LPS.During these three years ive almost the same problem that two times my reefs died due to cyano and dinos and i knew thee reason and i finally solved that.But this time some of my corals are not doing their best and one of them just died 2 weeks ago. You know what... some times my tank is very good and no problem is detected but suddenly i watch my reefs not happy and at last they die.
Ill mention my parameters and lightning,you give me feed backs .

Mg have not test it for a while but i dont think it be the problem.

I used activated carbon cause i thought sth has entered my tank that im not aware of,but it didnt work.

For lightning ive a 14000 metal h lamp with two rows of royal blue LEDs that i made myself.
 
The problem that i face was that i see some of my LPS's skeleton appears through the reefs mantle.

As you see in the bobble's picture a piece of it is bleached and i do not know why its happening:(:(:(
 
Have you tried dipping it? Looks like a bubble coral...how long have you had it? Do you live in an apartment or a place where you don't control pesticide spraying?
 
I try and keep my alk at 9to 10. when my alk gets low my lps start to shrink-up
 
How much lighting do you have? Seems like a lot for an LPS dominant tank

No i hadnt dip it recently.I have it for 6 months.Yea i live in an apartment but wew dont use these kind of sprays.
 
Looks like too much light lps need the least amount of light.
 
It was in ths flow for six months and it was very good.

Yes, I agree it looks very happy in that photo. Have you changed the lighting schedule or replaced your metal halide bulbs recently? When was the last time you changed out carbon?

My bobble was going to die about three months a go.I added ordinary fresh water to my tank (about 4 litrs that was evaporated)and it got back.How can i raise the po4?It was always near zero.

The metal lamp was changed a year ago.And i changed the carbon 2 weeks ago.But i took it out after a week.
 
My lightning program does not change very much but some times i turn off the metal to watch my tank and have fun(LOL)
