Lubbock's wrasse dead, why?

I got a lubbock's wrasse about 2 weeks ago and it's now dead. I fed it last night and I did see it eat so Im not sure why. Here are some pics

(White spots are not ich, it's on the ziplock bag)

Ammonia and nitrite are at 0 while nitrate is at 35.

I have a firefish that's still alive so I dont think it might be an aeration problem. Any guesses?
 
Sorry, not enough clues/symptoms to go on here. Did you see any rapid breathing? Is the fire fish breathing good? Did you quarantine the fish with any method?

hard to say with a dead carcass but can be a number of things:
Low Oxygen
False test readings (35 no3 and 0 ammonis doesnt balance)- Tells me API test kit?
Injury from being startled
Parasitical

Did you notice any heavy breathing ?
Was fish eating ?
Was it listless?
Any other unusual behavior such as darting through tank ?

I tested with salifert test kit. I leave my nitrate at 30 usually. Im not sure if it was injury from being startled since I was away from the tank for the entire day and no one else came near the tank.

I did notice somewhat of a heavy breathing (opening mouth every 1 second ish) when it was hiding, but when it comes out to eat, it acted like normal. Fish was eating (I fed it the night before).

I did see that it was "kind of" active when I first got it, but as time went out and kept hiding in my rockwork and only came out to eat.

The wrasse never darted through the tank or anything like that.

My suspicion is flukes or velvet in which both unnoticeable up front until it progresses.

I did see "rapid breathing" (opening mouth every 1 second) when I saw him in the rocks 2 nights ago. The firefish breaths normally. I didnt quarantine the fish, but I am going to set a quarantine tank up very soon because of this.
 
Ahh are there no physical symptomps to this? No, white spots or anything?
Eventually with flukes, you will see yawning effect , rapid breathing, scratching against objects, cloudy eyes and inflammed gills with refusal to eat.
Velvet evventually covers fish to naked eye , fish swims at surface or into current, loss of appetite, hiding, rapid breathing.

I managed to get some footage on my camera of the fish last night and saw this. Any thoughts? (look at bottom right) (This was around 1 am)

Very ingenious to check camera. If this was 1am, prior to that something happened whether injury, neurological or??? as in the video, fish is moribund- A term which is similar to coma state and is generally a point where fish is weakened and unable to eat or swim normal and may or not make it.
I dont see evidence of another tank mate suggesting aggression and it may have well gotten startled and hurt itself but hard to say without viewing it as it occured IF it did

I did not see it as a disease issue

no clue - but at first blush - it looks extremely emaciated - possibly in poor condition - and it could be many things depending on your tank
