Mandaren Dragonet dieing?!?!??

Ok so i have a 30 gallon biocube and its been running for about a year and a half, i have 2 clownfish, 1 fire fish, and a mandaren dragonet. since yesterday ive noticed odd behavior but only eith the madaren. I thought maybe it was the heat since its been really hot where i live! so I turned it down on my heater but shes still acting weird..I found her today and shes just lying there on the ground and her fins looked all shred up :( my only other guess would be that I have a LOT of bristle worms..? PLEASE HELP ive had her for a year!!!!
 
I would put her in a QT tank , verify she is eating (feed her or him copepods) let her heal then return her to DT after she has completely healed.
 
Mandarins are difficult to quarantine because of there eating habits. Bristle worms will not do that to a fish either. My two guesses would be

Your pod population in your bio cube is depleted.

A rock fell on the mandarin.

How much rock do you have in the biocube? Unless you have a ton of rock, constantly resupply your pod population, have a refugium, or the mandarin is eating frozen food a 30 gallon tank typically can't support a mandarin.

+1 also could any of your other fish be picking on it.. Bristleworms dont pick they devour fish so it wouldnt have survived.
