About two weeks ago foot fell off on its own. Seemed to be thriving before that.

It definitely looks stressed out. How long has it looked like that? How long have you had it? Have you checked it closely for pyramid snails? Do you have anything in the tank that might like a little nibble?

So many questions.....

IME, once they look like that they will either recover quickly or be crab/bristle worm food in short order.
 
Have had it for almost 2 years. It was in another spot for a good 5 months (tank size upgrade) and looked like it loved the new tank and spot, it was continuously growing then one morning I found it on the sand, foot was still attached to rock with bristle. Read that from time to time they shed the foot and it’s not a death sign. After that, I then moved the clam when that happen to a better light spot/current spot (bout two weeks ago). The current look is going on 3-4 days.

did a ruby reef rally for possible brook in DT.
 
If I tried to move it still is stuck. When I direct feed reef roids it closes... a bit
