Had this clam for slightly more than a mth. It never really had full extension. But last 2 days since moving it to the top and back to bottom, it became like this.
Current position par is around 120
No3 15 po4 0.07 all 8
Anyone know what is causing this?

How big is the clam? I'm not seeing new shell growth so it might not have been healthy to begin with. It's starting to gape so unfortunately they don't recover after that.

Most likely wild, they need a lot of light. Getting a healthy clam is the biggest hurdle IMO. Try Pacific east aquaculture, they have healthy clams. Unless you know it's tank raised or from a hobbyist, stay away from the large maximas.

That's the early stages of gaping, no bueno!
I'd stop moving it but, T maximas need lots of PAR, 300+ ideally.
difficult to tell exactly what's stressing it, moving it a bunch certainly doesn't help.
also good idea to check near the byssal opening for small parasitic snails and such.

Clam died this morning. Took it out n found this worm like thing on it. Can it b the cause? Any idea what this is?

Bristle worm, eating it after it was dead most likely. Although I have had some big ones that I swear nipped some of my clams mantels lol
