Meat coral skeleton help

Been having trouble with low nitrate and PO4 recently. Here is a pic taken tonight of my cynarina. Any input appreciated.

Nitrate 2-5
PO4 undetectable
Calcium 400
Alk 8.9

Using salifert

Tissue recession and not good
Some typical causes are:
High alk
High calcium
Too much flow
Too much or too little light
Pests
Temperature too warm
Salinity elevated

some things to check
Also look under the Coral for any pests
