It doesnt seem too crazy large. Is he eating like a pig if so that may be the reason for the belly. Keep an eye on it to make sure it doesnt grow otherwise it might be a cyst or tumor. I'll link an older post of a wrasse that had a cyst. Its stomach was probably twice the size. https://www.reef2reef.com/threads/wrasse-with-enormous-swollen-belly.448536/

Anyways i had a tailspot blenny that never stopped eating and its stomach looked huge compared to his body. Put him in qt where there isnt really anything to graze on and his stomach size went down

Also have you treated with any anti parasitic medication. Most parasites make them thinner but there are some that do the opposite

Focus and an antibiotic will help if this is an infection or parasite in the digestive tract. And some boiled peas may help if this is a blockage. The antibiotics i would use would be either metroplex or clindamycin. Both have anti parasitic properties as well as being an antibiotic. Clindamycin will have to order online and will be more expensive. Easier to dose as comes in pills(at least what i have from alivet).

I have API general cure on hand and Prazipro? Is that sufficient

GC contains metroplex and prazi so do not mix the two medications you have. When mixed with focus its 1:1 ratio, and a tablespoon of food.

You can also add epsom salt to the water(magnesium) will have the same effect as it does on humans and help to pass anything.

How i mix so the mixture lasts long enough for a whole treatment. This is alginate not focus so slightly different consistency than focus.

how much Epson Salt do I add. I would rather try that than the peas, just because I don't know if he will eat those.

how much Epson Salt do I add?

how much Epson Salt do I add?

If done as a 30 minute bath 1tablespoon per gallon.

if treating the tank 1 tablespoon per 5-10 gallons.

Its a wrasse, so a bath is easier said than done lol

I think we are jumping the gun on treatment here. I sometimes think we try too hard to treat everything. I'd start with just the general cure for internal parasites. Is your wrasse pooping at all? If not its probably a blockage. It's fairly common for these guys to develop a gut especially if you feed heavy and/or you have stuff in the tank it can graze on like copepods
