Melanurus Wrasse- weak but tries to feed?

I've noticed the last few days my Wrasse has been acting off. Staying on one side of the tank, swimming slow etc. Today he seems to have gotten a lot worse. He occasionally rolls around on the bottom and then seems to go back to normal for a minute. He has had a broken jaw for about three years now (I have had him four). Any ideas what could be going on? He has no sores or anything on him, everyone else in the tank is doing perfect. I did notice He seems to be swimming with his tail a bit lower? I did some reading and maybe he hurt his back? I fed the tank tonight and he went after the food but you can tell he's struggling.

His back/tail does look odd. Wonder if something happened to him. Also looks a little thin. Is he eating normal?

I put food in and he was trying to eat but is definitely having issues swimming.
 
Pinched in deformity mid-section of fish causing buoyancy issue. Possible internal parasite.
If you can get it to eat, Seachem metroplex with Focus as a binder for the food would likely help this guy

I have never had to give meds. Can the other fish eat it or do I need to remove him?
 
If he won’t eat, then you’ve got bigger probs.

If you get it to eat, you can get metroplex in its system
You would Feed the medicated food mix each day until the infection clears up. The remaining food can be refrigerated or frozen between feedings to preserve it.

This should be done in separate quarantine for the infected fish only
