Why would my mushroom look like this? I seen some sticking out it's mouth also.

I don’t have a bunch of experience with Yuma’s but mine melted away shortly after showing them out their mouths. Did you read the tacked Yuma thread? Might be worth a look. By the time I found it, it was too late to save any of the ones I had.
 
Generally ricordia and similar mushys lay flat on the rock.....he's stretching way out to get the available light. Which is what I think is causing u to see the filaments....what type of lighting is on the tank?
 
It a little little light that probably needs upgraded

It's like 12 watts

The tank is only a 5 gallon cube.
 
He looks good in that pic. Did you change his location at all?
 
Yeah I moved it up the rocks half way then it freaked out after a week of been in the new spot. Like the first pics. It's on a piece of rubble so I was able to move it. I now moved it back down in the tank hoping it will do better but I have a green one that I haven't moved and it seems to be doing the samething with the "pucking".

That’s how mine went. I cut a couple in half and those lasted a week longer than the ones I didn’t. Wish I had tried something (iodine dip, peroxide dip, etc) At least I could have said if they helped or not, I was hoping they would recover on their but they didn’t.
 
My system is going through a mini cycle. I'm I think it was due to feeding a new food for my NPS coral. I'm gonna use some smart start complete to help boost the bacteria. Hopefully that will get things back in line of course with water changes.

I finally got the chance to take my water and get it checked and I had ammonia showing up and my nitrates where high so I'm assuming it's going through a mini cycle the tank is like a year old. But it is only a 5 gallon tank.

Yikes! That'll do it. Did something die?? I would do some WC asap...might b too late for the mushies but will help everything else.

Nothing had died I just started to feed this new food to the NPS corals. I think it was to much bioload for the system.
 
Would it be possible for the dying Yuma’s to be putting off the ammonia? It could be the food, how much did you feed?

I was feeding it twice a day for like 3 days from start. Without thinking what I was doing to my bioload. Then I noticed the polluted water. I should have gradually added the food to the system. It's coralific delight
