Mimic Tang mysteriously died

Have no idea why, swimming normally, had no diseases, tank water is ok, nothing. Was on a mix of algae pellets, omnivorous pellets, nori and mysis

Water parameters are:
Salt: 1.022
Ammomia: None
Nitrite: None
Nitrate: 5ppm
Phospate: 10ppm

I have a big cave structure with lots of nooks and crannies in the middle and found him laying on the right side this morning. I had him for 3 months and my tank is 5 months old. I cultured my live rock to have plenty of algae in another tank and I also gave him a seaweed clip. He lives with a Desjardini, reared mandarin, longnose hawk, fiji foxface and an archerfish. BUT the desjardini was never aggressive to the tang, they often shared the same food no problem

All of them were at least 3 weeks in quarantine along with another week of quarantine in the store itself
