I picked up a monti frag a week ago and have noticed some antenna or tentacles popping out of where a polyp used to be. Any ideas what this is? Worm?

If bad, what's the best way to remove? Dip? Drop of superglue over its hole?

Looks like a spionid worm that the coral grew over. The tentacles that come out could irritate the coral so if you want it gone just put a drop of superglue on the opening.
