my BSJ needs help!

Hi everyone,

I've had a BSJ in my 34gal display for about 4 months now. 2 days ago I started to notice he was acting a bit odd. He wasn't really using his left fins, hanging out in odd spots leaning against rock work (uncharacteristic of his normal behavior), and I would occasionally see him dive at the sand trying to scratch his sides. His coloration was a bit dull as well, but he was eating normally like a pig. I let a day go by to see if things would improve, but they seemed to have gotten worse. He would normally swim in the water column interacting with the other inhabitants but he was by his lonesome leaning against a rock not swimming much at all. I've since relocated him to a small hospital tank for observation and hopefully treatment. I've had a hard time getting good pictures since the hospital tank has a curved front, and he freaks out when the camera approaches. Here is what I could get:


usually his back fins are up and keeping him steady in the water column, but now they're usually matted down like so:

he can still has full use of them when he freaks out and swims around, but in his natural state he's just sitting on the bottom next to a PCV pipe. He still responds to feedings though, which I take as a good sign...hopefully I'm not being too optimistic here.

I should also mention that the other 3 fish in my main display are acting normal and show no signs of distress/illness (2x ocellaris & a pygmy anglefish). Any idea on what is going on and how to treat it?
 
I forgot to mention that the lump on the right side of his head has been there since I've gotten him. I treated with metronidazole and it didn't get better so it seems to be inert and unrelated.
