Mysterious zoa deaths

At the middle of last year, I purchased some Zoas but they soon died off and I didn't think much of it as some purple star polyps had hitchhiked on the rock and had popped up a week after the death of the zoas. When the zoas died, it left like a white husk that stuck to the rock but was easily scrubbed off when I was cleaning some hair algae off the live rock. Today on my main rock work, I saw one of the white husks attached to my main rock work and I have no idea why it got there as I thought I had removed all of it. Has anyone had zoanthids die and this happen to them? I will get pics tomorrow as my phone has no battery and I'm very tired.
 
We need a LOT more information. Tank parameters (and what test kits) age of tank, lights being used, what if anything are you dosing.
 
Light are Aquaone Mariglow 60, not dosing anything yet, Using API reef master test kit and API Saltwater test kit, pH of around 8.2, 0 ammonia, 0 nitrites, around 0 nitrates, 340 calcium, 9 dKh. Currently the tank has a Fire shrimp and 2 medium-sized clowns.
