Mystery wrasse problem, please help

My mystery wrasse has been hiding under a rock piece for the last couple months, don't really come out swimming much. But at feeding time, it did look out and grab a big piece of frozen then went back, much like my marine betta. Couple days ago I was doing maintenance on the tank and moved its rock, but this time it didn't go back to it afterward, and just hide under a small piece of rubble nearby instead, and didn't move at all even at feeding. I noticed its right eye seems to popped out a bit, like drop eye. Yesterday I tried to catch it out to see if I can do something about it, and managed to catch it bare hand ... Now it's in an acclimation box in the display tank.

What does it look like the problem is? What should I do next?

That eye looks as though it's taken quite a whack. You may find that epsom salts (USP Grade, no additives) helps to reduce any swelling. That treatment is reef-safe, but I'd be pretty surprised if you didn't eventually have to move him to a quarantine tank for treatment with antibiotics.

Additionally, he seems to have some white poop going on. If that gets stringy, he's likely dealing with some internal parasites, as well.

So move it to a quarantine tank, and treat with epson salt? How much to use? Should I run some antibiotics too? the same time with epson salt or after? which kind?
 
Yes, I would advise the epsom salts and the antibiotics. Make sure metroplex is part of that just in case that white poop is because of intestinal worms. You can safely combine metroplex with kanaplex and furan2.

Metroplex is for internal parasites. But it is not eating at all now, should I wait for it to start eating then treat with Metroplex?

I also have PraziPro at hand. Will it be useful in this case?

Yes both are antibiotics, combined they widen the spectrum.... when you add in metroplex it widens it a bit more. Yes metroplex is primarily for intestinal worms and works best when eaten, but fish also drink the water they swim in so some of it can reach the gut where the worms are. The hope is that enough of it is drunk to allow the fish to start eating again so that it can eat the medication with it's food.

If you already have furan2 then you could treat with that and safely combine it with metroplex.

I just started with Furan 2 in quarantine. 10 minutes later, it lies down on its side, breathing rapidly. Occasionally dash to the top of water. Is it normal?

Medications can suck the oxygen out of the water. It's not usually all that bad with just one med, but you may want to add an air stone or small powerhead pointed at the surface of the water to the QT to increase the O2

It's running with a DIY air driven filter. I just added another air stone as well. Hope it will make it.
 
Finished first round of Furan 2, did a 25% water change, started the second round.

I also added epsom salt the first night.

It looks definitely a lot better now. The eye looks recovered. It's able to hold itself up without leaning on something. The first day it was lying on its side. Breath normal. Still not eating though.

I ordered Metroplex. But still couple days before it will arrive. I'll do it after finishing Furan 2.

For espom salt, it doesn't get consumed like medicine, so no need to keep adding, just add back the amount removed by water change, correct? Should I continue to use epsom salts during Metroplex as well?
