I have a naoko wrasse that is about 3 weeks old in my tank. Was perfectly fine yesterday today i get home from work and he is swimming in cork screws and running into things but then will stop and swim normal then it will start swimming all crazy again sometimes jerking. All my other fish are fine. I dont see any strange spots are wounds.

Ideas here? Am i gonna lose this fish?

#reefsquad any ideas? Can you give us some more information? Parameters, was it quarantined prior to adding to the tank, have you added anything else to the tank recently?

Parameters and recents changes might help, but this kind of thing seems ridiculously common in fairy wrasses.
Especially recently, it would seem. I've seen quite a few recent threads about neurologically impaired Cirrhilabrus.
Sometimes ascribed to a spinal injury? I feel like there's more to it than that, but no clear pattern.
Prognosis isn't good, unfortunately. Some would recommend isolating it in an acclimator and adding epsom salt to its water.

Can you post a video? It doesn't look good in the pic.

Is it still able to eat? Do you have an acclimation box you could put him in?

Humble has mentioned recently that he thinks some spinal injuries may actually be the result of internal flagellates invading the spinal column. If he’s eating and you can get him somewhere safe to pipette medicated food to him, he might have a very slight chance at recovery.

Once it gets to this point though, prognosis is not so great :/

There has been some evidence with wrasse that internal flagellates can cause neurological issues that appear as spinal injuries. I think I would try metro or GC laced foods for 14 days as long as it’s eating.

I didnt see the parameter request.
temp 78°f
salinity 1.026
ph 8.0-8.1
nitrate - 30ppm (higher than I would like)
phosphate - 0.001
alk - 8 DKH
calcium - 410ppm
mag - 1300ppm

He was quarantined for a week.

So internal Dino's? Something that came from my tank? This really stinks. First fish I decided to spend a chunk on...

Not sure, but other wrasse have become victim of “spinal injuries “ without injury. This is probably from an internal parasite building to the point of a neurological disorder.

So stay away from expensive fairy wrasse' Cool thanks!

...Or feed them medicated food upon arrival as a preventative ;)

This one doesn’t look so hot, but if you can get him to eat at all metro (or General cure) + focus (1:1 ratio) soaked 20-30 minutes in your choice of frozen food. Feed small amounts (as much as they will eat without too much “waste”) twice daily, for at least 10 day, or up to 3 weeks.

Well i will get some metro tomorrow on the way home. You are talking about metronidazole correct? See maybe if it will help i dont know if its eating though

So i have been thinking of how to get some of this metro and I just remembered my wife had gallbladder surgery a couple months back and has 9 500mg tablets left over! So how exactly do i dose this out?

I have a scale that will measure that small. How much do i soak with food? Also if he is not eating will moving him to a separate small tank do any good? Is the only way yo get this stuff in his body is by him eating it?

So how is/was he? Did he pull through or is he up in the "reef in the sky"?

I tried my best to but in the end it was too late. I couldnt get him to eat....

It unfortunate but its a lesson learned as well. I told the guy I bought it off of a d he said he had never heard of that before. So we both learned something
