Naso Tang QT help

Hello my fellow reefer friends, this is Not an emergency, We have the Naso at LFS, been there 2 1/2 weeks at store. For years I’ve followed sick fish treatment and my mind is literally blown, looking at him I think he’s got everything so I need opinion (will gut load with clams via PaulB’s advice, love that guy)
I have a bleached, prime cleaned 20GL ready with seeding sponge and ceramic balls in sump ready for sterile aquaclear 70 (can’t autoclave it!), Heater, powerhead
She looks bad as of yesterday and owner is kind enough to keep her, she did eat brown algae. Here’s pics ANY advice is So appreciated, also have Nuri-Sea water, about 9 gallon out of 20 to use.

Forgot to add, husband saw maybe ich on her fins, owner said he’ll treat with Ruby Reef.

Hi, I’m not sure I understand the situation- is the LFS holding your fish for you or are you going to buy this fish? My rule of thumb is to avoid acquiring any fish that is “ADR”...Ain’t Doin Right. If a fish isn’t perfect, I’ll take a pass and wait for something better. Fish disease issues are so difficult to manage, you certainly don’t want to start off with a problem.
I can’t clearly see anything in the photos, just one white spot, and the eye looks a bit glassy.
