Need help diagnosing longhorn cowfish

For starters, I sold this to customer about 2-3 weeks ago. She has an all in one tank (20 gallon). He is one of three fish. He was the last addition. She did not Qt and now has noticed what she thought was flukes. I recommended two things . Prazi or freshwaterdip badged on pictures I saw. Fresh water was her choice of remedy. She dipped for 3.5 minutes and noticed what she thinks was a few flukes come off. She stated he eats well and does not appear bothered but to me sonething is off and want to help her. Her tank parameters are 20 gallon AIO, salinity 1.024, temp 77.5 , ph 7.9. She changes 2-3 gallons of water every Sunday. Please look At photo and see what you think and recommend

The minimum tank size for a longhorn cowfish is around 250 gallons!

I would do 2 rounds in Prazipro in a separate hospital tank if flukes were found in the FW dip.
 
I've read that they can go from 2" to 8" in a year's time... they grow faster when they are small. They are also venomous and can release toxin when threatened that can wipe out a tank.
 
Yeah I’ve had several of these guys and put them in many maintained McEnroe account tanks the biggest one o have is 3” and he’s been in that tank for over 3 years. I’m just really struggling diagnosing this issue It looks more like skin coming off then flukes but can’t really tell for sure
