Need help, Duncan colony and hammers dying

My Duncan. Colony and hammers are dying slowly. All other corals are fineMy parameters are:
pH - 7.8 pH
Alkalinity - 8.4 dkh
Calcium - 475
Magnesium - 1513
Ammonia NH3 - 0.1 ppm
Nitrite NO2 - 0.1 ppm
Phosphate PO4 - 0.16
Nitrate NO3 - 25 ppm

I know my nitrates are high but don’t know much about the phosphate.
What would be your suggestions?

When my dunkens were dying/retracting.. It had brown stuff all over base and it was slimy. Found out it was some type of bacteria infection according to forums. Does yours have that?

Anyways, from what I read it was best to remove it right away so it wouldn't spread on other corals. It sucks.

What fish are in your tank? I had. I was puzzled on what was killing my Duncan and killed my hammer. I thought the hammer was polyp bailout but later realized my filefish was the culprit.

I have a yellow tang, eye kole tang, coral beauty, two clowns, a purple fire fish and a royal gramma. I never see anyone picking at the corals.

You shouldn't have ammonia or nitrites in your tank. U added fish before the cycle was done?

How old is the tank?
What is your salinity and have you double checked it with a second measuring tool?

I have this tank running for 3 years and no problems.

3 years and salinity is at 1.026

I just had a five year old colony croak on me, about 30 heads, but everything else is fine, so I shrugged it off.. corals dies every now and then for what appears no reason at all..

Whatever you use to test salinity, calibrate it and test again or use a separate device
False salinity can do this.
These are fairly easy coral and the main requirements are:
Moderate light and water flow
Temp 77-79
Ph 8.1-8.3
Salinity 1.025
Ammonia < .03
Nitrate < 04
Phosphate. < .04
Feed 2-3x per week

I don't know what the problem is, but it does not lie in the parameters you posted.

Tap water or RO/DI?

Any chance something metal got into the water?

RODI 0 tds. I don’t think any metal get into the water but I will take a look in the tank and sump just in case.
