My goodness- How long has this been progressing as this is not an overnight occurence? In addition to ich, it has a heavy bacterial disease and needs immediate treatment if its not too late.
Quarantine will be needed and treatment will be both Seachem Kanaplex and assuming fish is eating, seachem Metroplex added to food at .5gm weight which is a Very tiny amount.
Water quality will be of utmost importance and monitored with a reliable test kit

I would say 2 days
I put him in the Qt tank for the ich
Came home yesterday morning and he looked like this

What other fish are in the tank with it…either in this tank or the tank it came out of?

There is a yellow tang, 2 damsels
And a hippo tang- but it died after being placed in the quarantine tank
I took all the fish out and treated them for ick. All but the purple tang look fine

Can you supply some of the basic info to help us understand and diagnose. The condition of the fish is very disturbing to say the least.

So, what are your water parameters:

ammonia
salinity
temperature
pH


Did you use RO/DI water?
what testing devices are you using?
have you already dosed cupramine and how much?
What visible signs of distress were present when you got the fish?
Be sure to answer for all of the fish you placed into QT at the same time.
Did you buy the fish at a LFS or mail order?
How did you acclimate the fish?

What was the appearance of the hippo when it died?

I wonder if the yellow tang thrashed him and then a secondary bacterial infection set in?
