Need help. Something wrong with my SPS..

I haven't been able to successfully ID this one frag.. But I noticed today there is some tissue recession.. Help me please!?! I'm not sure if there is a way to save it or if its a lost cause...

This is what it looked like when we got it.

After a couple weeks we put it on the rock work. It slowly lost some of its vibrant blue color and the polyps started to come out less.. But it was still growing.. I don't know what to do... Please help.

Our params are
Salinity 1.025
Alk: 10
Ca:460/480
No3: 0
Po4: 0
Ph: 8.4
Temp: 77.8 - 78.3



Side note: While doing some more research we thought it looked closest to an Acropora Formosa..

looks like rtn cover the white part with superglue up onto the flesh of the frag
 
Thanks for the reply! I had to do that with our shortcake last week and didn't get it all.. :( lost the ssc in less than 3 hours :(. Hoping to try to save this one too..
