Need help with a Royal Gramma

This is my third Gramma. I haven't had good luck with them. The first two died abruptly in QT, now this third is having issues.

Anyway, he's lethargic and not eating well. I've had him a week and just today he looks worse. He looked great in store. His body looks good but around his face and one eye it's looking cloudy and puffy. I cant get a good pic. This is all I got. Please help diagnose and how I should treat. Thank you!

It's a 10 gallon tank and I just did a 50% water change 2 days ago.

I'm thinking about getting some Furan-2.

I will add he has looked okay up until today. He is out and swimming regularly but also hides a lot with is expected. He eats but not great. Likes mysis the best but would only eat a couple and stop. I haven't treated him with anything else yet. Just this evening I notice him looking more lethargic, lying more on his side.
 
The cloudy eye plus the marks on face look like flukes are at play here.
 
Has he been through any treatments? How long have you have him? Was this one in the same tank as the previous two that died?Flukes can cause a cloudy eye especially if it just in one eye.
To check for flukes perform a 5 min FW dip as instructed below.

If there aren’t any flukes I would dose the trifecta as described below for a bacterial infection.

I've had him just over a week and have not yet treated anything because he hasn't eaten as well as I hoped. I have Prazipro that I usually treat for flukes just in case. He is in the same tank that the others were, but I haven't tried 3 Grammas in a row. I've successfully QT'd two other fish since my last Gramma I tried.

I'll freshwater dip when I get home today and start Prazi if I see any.

I did a freshwater dip and revealed flukes I think. Started Prazipro.

Yes, those look like flukes. You should also start the antibiotics (trifecta) to treat for a secondary infection that is sure to follow.

Yes, just make sure you aerate the water heavily. Do you have Kanaplex?
