Need help with my tang

I have a sailfin tang and he cant seem to swim he trys but stays on his side any idia whats wrong with him?

I have had him about 2 months i did fresh water dipped him and tiny black spots came off him
He is in a 10 qt right and yes he has been breathing hard..

I'd FW dip him again and then treat asap with General Cure or Prazipro + Metro to completely deworm and rid him of flukes. Just be mindful that the dip will only relieve mostly skin flukes. The flukes, if they are there on the gills, need the GC or Prazi to help the fish breath.
