Need help with regal angelfish!

I purchased a regal angel about 1 week ago from a local fish store, I slowly drip acclimated him into my aquarium because I didn’t want to stress him too much. After a couple of days in the tank he began to develop a sudden twitching of the head/body also rubbing against rocks and sand. There seems to be some stuff on its fins as well.

My ammonia and nitrites are at zero and my nitrates are at 5ppm. I will attach additional photos for you guys to see and make your own judgement on what his condition may be. Im feeding him rods marine food and he doesn’t pick at it too much but I recently started to feed clams and he picks at that much more.

I am debating on between keeping him in my tank or trying to feed him with additional vitamins/ supplements, or moving him to a quarantine tank to better monitor him, I just don’t want the move to the quarantine tank to stress him out even more and cause more problems. Any advice and help is more than welcome and much appreciated.

The behavior you describe could be flukes. The first step would be to either confirm or eliminate flukes as the problem. So a freshwater dip should be considered. It looks worse than it is. Very common first step for treating potential problems:

Would you recommend put him into a quarantine or back into my tank after the fresh water dip? Ps the quarantine has been set up for a week it is fairly new
