Need urgent help for my hammer please!

Not sure what your question is, but number one problem with hammers is low or fluctuating alkalinity. Medium lighting and medium/high flow.
 
Sorry for the incorrect and doubled pics. I have bought this frag last week and it was doing fine but today half of it is brown and closed as you can see from the pics. My parameters seems fine my nitrates are 15. I cannot figure out what the problem is.
 
This is how it was till yesterday. Thanks Mike, my alk is 9 and lighting is by leds, it is half way down and my tank is shallow, is it possible that it has alot of light? My leds are mariglo, I don't think they are too strong.

From the green background i see no3 po4 anything above 5 will have affect on polyp extention a lot of new import got flatworms
 
It could be brown gelly disease u need to dip it in idoine based coral dip check it for bugs and do some big water changes
 
Those tube worms that spit out the slime web those u can put glue over them coral dont like em
 
That's it! Lost my 8 head torch coral to it. My understanding is to cure dip it in lugols. I have read you need to dip it every few days once will not do it. If it is Brown Jelly they go quickly. Hope someone else can chime in to confirm regardless lugols wont hurt it anyways if used right.
When you say brown, could it be brown jelly disease, which is common of euphyllias.

There was a tube worm for sure which was touching the hammer coral with it's slime web ! I had no idea that it will do any harm! I moved the hammer to lower part now away from the worms. I hope that if this is the case it will recover! I will see how it goes. Thanks for your great help and advice. I will keep you updated.

unfortunately I don't have any coral dip since I am totally new at reefing and this was my first time and was not prepared for a bad thing like this. Tomorrow I will go and equipp my self. Seems that the bad things happens very fast in this hobby. The coral seems very bad this morning and I only can buy stuff tomorrow! Thanks again for your help.
 
+1 on Brown Jelly.
I'd take it out making sure not to let the brown stuff release in the tank. If it had multiple heads then I'd syphon the infected ones out so they don't infect the other heads or other LPS in the tank. A Dip also helps sometimes yet most of the time infected heads are lost very quick.
 
Brown jelly in some corals is a sign it's a goner. When you see the effected coral try and remove it for cleaning and dipping. Being careful not to allow any of the jelly to release into tank. This may effect another coral. I never had luck if iodine dips and brown jelly. There may be other dips with a better track record on it. I had some luck with a peroxide dip as a last resort. A good dip once to twice a day for about 5 days. That is still not a sure bet. Be sure your water perimeters are good and stable. That will help bolster the corals resistance.
