Also I am curious, how could it get it's mouth into a power head and pull back out? I cannot imagine a Nem pulling itself back out of a power head like that, what species is it?
Weird, he's been in the tank for a month or two but I just got a new light a few days agoHmm I cannot see any tear on the picture but they can easily recover from a tear.
They can look really weird over a long period while adjusting. They will look great one day and then suddenly shift their "attention" to securing their foot. Then they kinda let go of the energy in their upper parts while getting an optimal grip, but I am not sure if that could look like a tear or similar.
It looks like a small specimen, that means it can run out of energy midway in doing something and that looks very flat so to speak.
How bad did the tear look (if you don't mind that I ask) ?

They will sometimes make that big bloated looking foot, either to move around or to get a better grip on things.
I suggest that you keep a really close eye on the tear. Nems does not have a central nervous system, so it cannot receive a damage that it cannot recover from.
Anemones can practically re-grow out of a left over tissue of anemone
The main concern would be, that when an anemone tears, it can begin to decompose.
So I would presume that your Nem have only smaller power reserves and since it had new lighting, it reacted by trying to find the optimal position. That was possibly too hard for it and it ran out of energy during the night (they kinda relax during nighttime, they also use that laxness to move around during nighttime).
If the Nem can close the tear back up, as many of us have seen happen after power head accidents, it can easily recover.
If you notice the Nem tearing more and more, it means the Nem have begun to rot and then it becomes to risky to keep it in a tank with other livestock.

it does look like that but this morning he was just sitting on some rocks it was severely in my softie side of the tank with cheap light. I moved my kessle over a tad, I didn't want to blast it but I wanted it to get atleast a little light that will actually sustain it. It looks like it was trying to climb up higher to the light.Is is climbing around? It looks kinda as if it is trying to split down the middle using the rock formation.
will have to decide which you like better.
