I've had this for a while, I think it may be some sort of sebae.. it's flat, and the size of a CD-ROM, maybe a tad bit smaller. it also always resides under a rock, mid tank, and in the sand bed. I also believe it's pretty aggressive!

^ Second Sebae.

A Condi is not as broad, fewer tentacles, longer tentacles, narrow base/column.

Google images of both and you'll see the differences are distinct.

Looks like a sebae nem to me
that's what I think. it looks similar to the "white" one on live aquaria, but it's never balled up like I see the ones at work do. it's always flat, which is what I find strange!

that picture was from after I moved, and it wasn't very happy. I can try and get one of it now. but I do bet it's a sebae.

BTW, there is no such thing as a "sebae" anemone. That's just a generic hobby name for both H. crispa and H. malu, with crispa being what most people mean by "sebae" anemone.
Just based on that pic, I would guess that yours is a malu, but stressed, small, or newly acquired individuals can be hard to tell apart from crispas. There are differences in the column that help with ID's, and given time and proper conditions, a crispa with have longer, more numerous tentacles and will get MUCH bigger.
