New fish with bumps, ich or stress?

Hey all I'm having a bit of a nightmare moment here. I had ich from an online coral I bought almost 5 months ago now, it wiped my tank and I lost everything! I left the tank fallow for 4 months just to be safe and I put my first fish a two barred rabbitfish in on Monday. I got him from my LFS who's always sold me healthy fish and do an in house quarantine.

However today I just noticed white bumps on him!!! Not a lot maybe 5 or 6, but I know what ich looks like and this looks like it. The fish was very stressed when he first went in, he chased his reflection for 3 days nonstop, from when the lights came on to when the lights went off. It's only been just since yesterday he's calmed down and is swimming around the tank.

My question is, could these bumps just be from stress and I should keep an eye on it and they may pass? Or is this ich, time to go red alert, fish him out and get him into a hospital tank? I'm asking because I have a lot of rockwork and he's very fast, not sure if I could catch him, and if I did would I stress him even more moving him to a tiny hospital tank?

I can't tell if I just have PTSD from my previous experience with ich wiping my tank. Or if I'm just MAJORLY unlucky, I got a fish with ich, and I need to act now. Thoughts?

Rabbitfish will often have weird looking lumps and bumps and that may or may not be disease.

Really need to see it to be sure one way or the other. As mentioned above pics would be helpful.

Yeah sorry guys guess it was silly to not post a pic. The one on the tail fin is a lot more pronounced, and I couldn't get the right angle to show you the 2 on his side fins.

It does look like ich from what I can see.

Did all the spots pop up at once? Overnight?

Honestly I can't say, I was looking at him closely the first 2 days but the past 2 days I'd been in a bit of a rush. It was only today I really noticed them.

Would lean toward ich as well. Can't rule out flukes from that picture, though. I'd recommend a FW dip.

I had some PolyLab Medic in my storage so I dosed last night, and again this morning (as per instructions). Much to my delight I don't see the spots today. I don't know if it was ich, if the Medic worked, I'm just happy to see him clear. I'm going to continue to dose the Medic as instructed for the full time, just in case.

However this has really gave me a reality check about quarantining. When I lost all my fish last time it was so stressful and so sad to watch them all die. I SWORE I would quarantine all my fish going forward. But after being fallow for 4 months, then another month before I finally got fish, I guess after 5 months the sting of losing all my fish had faded. But I tell you I was up all night losing my GD mind thinking I had ich AGAIN right away. It sure put the fear of god in me. I don't care if it's another expense I've ordered my QT equipment today!

Personally, I would set-up a QT now before you have a tank full of corals and other fish. I don’t trust anyone’s QT except my own, and this is exactly why.

The medic is a great management tool, but not likely to fully eradicate ich from your tank.

You have a choice to make, treat this fish... go fallow again, and QT/ treat all fish moving forward to maintain a parasite-free tank. Or, run your tank in an “ich-management” style, using various management tools to keep the parasite populations in check.

There are multiple threads here on both methods, I’d read a few of these before making your decision.
