I'm sorry to be so ignorant, I'm just now starting/planning a course to take in QTing fish.
Would you mind explaining your QT method?
Do you WC? If so how much and how often.
Do you immediately introduce meds to treat them?
Do you use shorter light periods to reduce stress?
You said you use an air stone for oxygen transfer. Do you also use a circulation pump?
Any help is greatly appreciated. I've been just acclimating my fish to my DT and have been lucky so far. But that's gotta stop.
Here is a good place to start research, but keep in mind there are about as many different views about QT as there are reef tanks out there.

I use a method similar to the one outlined in those links, by @HotRocks . Although, I usually allow an observation/acclimation period of up to two weeks prior to starting prophylactic treatment, unless symptoms crop up beforehand. I like to get the fish comfortable and eating well prior to medicating if possible.
Of course, sometimes you just have to tackle each event in QT as it comes...
