Thoughts? Came home and green bubble tip was laying in the sand upside down. Can't see a foot? Dead? It's been in the tank about a month. Have a RBTA that's doing great.

Well, they will always look cruddy out of water, but for the most part if it's still solid and not mush it still has a chance, but the fact it was not set and you found it on side of course is an issue.
It's hard to say from this end, I'm a never say die guy so I'd be tempted to put it back, or maybe better QT it, maybe read up on treatment options and post pic of what it looks like underwater in QT
 
Also you should probably tell us about your tank, specs, params, history, time running and all that

I don't test alk or calcium.
I think it might be the Peppermint shrimp. Saw them messing with it
 
Well, you should be testing more especially alk, and that size tank is a little tougher to keep stable as we'd like, it's also on the smallish side for more than one nem IMO, close to intakes and such.
It would be nice to have seen the nem in question in tank to really see what it's doing, also lighting, what are your nitrates, filtration, so on.
