New Worm

Is this a bobbit? Haven't added anything new to this tank on like 4 years.

No, not the infamous Bobbit Worm (E. aphroditois).

Most likely a worm in the Eunicid family (or one of the similar families) of which there are hundreds of species. I have at least one really small one in my sand bed like yours and it does no noticeable harm. I have one in the rock work, too, that nips my hermits when they get too close. Most are detritovores, though some are carnivores/herbivores.

Is there anway to tell what kind it is? I am a bit semi concerned since it's in my Nano tank and have quite a few small fish in there.

Most likely a scavenger/detritivore. They spend a lot of time keeping their burrows clear of debris.

If it hasn't taken out a small fish in the last few years, then it probably won't. The largest one that lives in the rock of my 12g hasn't been caught bothered any of the small fish I've had over the years (only one cleaner goby has ever disappeared without a trace, but there's no evidence that it was the worm's doing).

If it still bothers you, you can set up a very simple 'pest aquarium' as they are interesting creatures or give it away.
