Nitrate problem???

Ok so I have a 29 gallon salt water tank I have had it since August and haven't had a single problem with it until recently.
A few weeks ago I had a spike in nitrates. Like really bad it was at 160 ppm and I had no idea why. So I did water changes every week for a few weeks and cleaned everything up and got it under control. A shrimp was dead and I'm not sure if he died because of the nitrates or he was the cause of them.
I finally got it under control and everything was great except for a mushroom and a few other soft coral that didint make it.
I am now having the same thing but nothing has died yet but I did have to take out a mushroom but he never really was the same after the first time.
I noticed a few day or 2 ago that my leathers where not looking so good and did a water test. And sure enough the nitrates where crazy again.
So yesterday I did a 50% water change and a few weeks ago I got the CPR HOB refug so I decided to put it on last night while I did my water change and got that all worked out.
But I am still wondering what could be the problem with nitrates. I have a pawer head in there but I'm wondering if I don't have ebough movement under the live rock and stuff is getting stuck under there.

But he wasn't dead that long he still had full color and everything that's why I don't get it
 
What is your water change schedual? How much do you feed? How many fish?

There are a bunch that could contribute to the nitrates

Are you over feeding?

After the first nitrate spike I cut down food a lot and i started Doing 50% water changes every week then after i got the nitrates under control and then started doing smaller ones every week then after a while I started doing 50% every 2 weeks.
All I have is 2 clowns 2 clown gobies a lawn mower and a diomond goby.
 
Are you using rodi water? When I first started I made the mistake of using tap water and my nitrates stayed at 160ppm.
