.....how can can you ensure a completely disease free tank? I get that fish need to be QTd' but what about other additions, should crabs, snails, inverts and be left in a copper free QT for the full lifecycle of parasites before going into the DT ?

If you QT everything you've got a pretty good chance of a disease free tank. It isn't a guarantee, but you've got a better chance. Here is a link to a QT guide for inverts and corals:

Unless you set up a completely virgin tank.... dry sand and dry rock.... and quarantine everything from snails to corals for at least six weeks there’s going to be something in there that’s detrimental. Even then there’s no guarantee.

Quarantining everything "wet" that goes into your tank is the choice you have to make. It will slow down the "build" of your tank for sure; but you will have a happier, more disease free tank = less worry
