Octospawn help

So it looks like my octospawns head may be splitting and it looks like some tentacles are falling off. On the other head though everything looks fine. Is this normal or should I be concerned? Parameters

Nitrite 0
Nitrate 10
Calcium 430
Alk 9.9

Maybe I need to feed it? I only fed it once but im not sure it was thriving a week ago I dont know what happened

That head looks like a goner. Fighting something similar and can't figure out the problem.
