I noticed this growing underneath my monti cap and at first I thought it was coral until I noticed that at the end of the spiral something kept moving it looks like a worm of some type. Is this something to worry about?

Looks like the tube of a vermedit snail. Generally harmless but can bother corals if they multiply quickly. Best way to get rid of it is to put a small dab of glue on the opening of the tube, and it'll starve out.
 
I had one that big. Yo could see his face in the tube.

I wound up naming him.

Some but not all corals can get irritated stung and killed by the mucus web.
We have some large ones in CA, namely Thylacodes/Serpulorbis squamigerus, whose tube openings can be as wide as a dime
