What exactly is a sea swirl? Is it a return that randomizes flow back into the tank?

Yup, it is a box that fits on top of your tank on your return line and oscillates back and forth via a motor inside.

I had one of these on a clients 180g tank. Truth be told, I got better coral growth with this than I do now with Tunzes. Powerheads are linear, although you can slosh them to lift detritus, where as a sea swirl is oscellating and every nook and cranny will get flow.

They are installed like any other dedicated closed loop and plugged in. They look fancier now than years ago when they first started.
