I'd say that I've been having the fish for about 2-3 months, that was the most recent fish added...The dottyback is breathing too deeply and too fast overall. The left gill cover is extending further than the right, but I think the real issue is the heavy breathing.
How long have you had the fish and when was the last fish added to this tank?
Gill flukes are one possibility, as is bacterial gill disease (less common).
