orange shoulder tang looks sick

so my tang has been getting skinny fast, have notice anything werid like strings hanging from him when he poops. but im noticing his head is starting to look black. i have a frost bite clown and he isn't having any issues. i do have a bubble tip anemone, so idk if he might of been stung.
 
seaweed on a clip, frozen angel and butterfly by San Francisco bay brand. this is what they gave me when i bought it.
 
Do you have any pictures? Feeding twice a day she should be good and plump. Internal pathogen would be my guess.

im trying to get a pic of him, but hes hiding under my live rock. he is a baby tang so he gets into the smallest places.
 
is there any way to tell if he has been stung?

Sorry but the diet is inadequate and the angel formula comprises of algae and sponge.
At Minimum, Feed the following:

Nori seaweed basted with Garlic
Spirulina Brine shrimp
LRS Herbivore diet
Formula 2 frozen and flake
Plankton
Mysis shrimp

TWICE A WEEK, ADD EITHER GARLIC EXTRACT AND VITAMINS SUCH AS SELCON TO ITS' FOOD ALSO

Great well I have some frozen brine shrimp as well. That I feed the tank with, but right now he’s laying down. So I’m assuming he won’t be lasting much longer.

Laying down and breathing fast is generally an indicator its expiring to death. Hopefully not the case

from these pic can you tell if its marine velvet? if it is i need to get a qt tank for my clown.
