Orchid dottyback issue on tail area

Hey everyone so my Dottyback has been in quarantine for a month with coppersafe. He swims into the fake plants I added at night I’m not sure if his infection has come from the plants rubbing on him or something else. No spots or anything else just what looks like a scab and missing scales by the tail

Ok now his tail is being eaten away like a shark took a bite. Please help
 
Thank you for the replies I didn’t know a therapeutic range of coppersafe could do this. I was stumped. My dotty is withering away and I’m afraid it’s too late. My clowns are doing great and are due to go into the mai. Display today. Thanks for the info it’s been a tough learning curve for me this year
