Orchid dottyback not doing so hot

Need help trying to figure out what’s going on with my dottyback I got home late a couple days ago to it laying on its side and immediately put it in a breeder box to keep everything else away from it it’s left eye is quite cloudy and it’s BARELY eating hopefully can treat it some how :( it also looks like there is a gash of some sorts on it’s head

I think I see some dots on the head… whats in the tank? He looks super beat up. Tagging @vetteguy53081 @Jay Hemdal and @MnFish1 who are fish medics to help further! They will want a 30 second video or longer under whites and all take parameters and info about whats in the tank already

I’ll have to run and get my water tested at my LFS but all I have is 2 clowns and a watchman goby other than that it’s all inverts so I’m not sure what could have happened
 
it looks beat up and diseased and popeyed, poor fish.
How old is the tank? What size is the tank?

Sorry to see. Looks like a bacterial issue stemming from injury or even bite marks . Isolation was good but now you want to move fish to separate tank for treatment using seachem neoplex

How old is the tank? What size is the tank?
It’s a relatively small tank around 30 gallons about 6 months old I’ve added each fish in slowly with the clowns going in first followed by this dottyback and then the goby

Good thing I have some neoplex on hand from my freshwater days it says it expired 4/22 should that be a concern? I think I used it once when I got it. And any advice on setting up a hospital tank because I currently only have 1 system

Set up what is eventually going to be my Pico reef jar with water from the main tank and a small rock from the main tank that I planned on taking out and moved the dottyback to the jar with a heater and bubbler and treated accordingly!

The fish's coloration is very pale and the fins look damaged. What other fish was it in with? Is it breathing rapidly?

It is breathing rapidly if I had to guess it was the watchman goby that attacked it since he decided to combine hidey holes with the dottyback the clowns show no interest in messing with it
