Pacman Dying?

This coral was healthy just a couple days ago and now it's half gone... any idea what could going going wrong and how to save it?

What are your parameters, did anything change in this week? Are you feeding, if so what and how often? What lights are you running what’s you par? Did the temperature change if not what do you have it at?

I do not but was going to ask about flow.

Is the coral getting flow
Is there direct flow to the coral
Is flow alternating and direct flow was missed and happen as a result

Only reason why I'm asking about flow is because it looks like via the image there is exposed skeleton. Like the flesh was removed. Sometimes this can happen with too much direct flow.
 
Is that a Galaxea? Looks to me like that frogspawn is sending out sweepers and stinging it. Normally, I would think Galaxea would win that fight, but if flow is in the frogspawn’s favor, the Galaxea could be defenseless. Try giving them some more room.

Here's my lates parameters:
Temp: 77.22
pH: 7.8
NO3: 16
PO4: .12
SG: 1.024
KH 7.0
Ca: 215
Mg: 1400

Flow is provided by two opposing 4K Gyes mounted vertically in the rear corners in an alternating flow pattern, plus a 3100GPH return (16ft head due to the remote sump)

I did start testing a Maxspect Razor r420 160W light on Sunday but didn't think this would cause this kind of deterioration... It doesn;t seem to far off from my other light which is a Aqamai LRM. Was hoping to get a better spread from three pucks vs. the two....?

Seems like the sweepers as suggested could be the cause (although I havn't seen them) so I've gone ahead moved it to a more isolated spot...
