Fish is starting to scratch, has some type of scratches in the black coloring of the puffer. Eating well, seems happy. No rapid breathing.

Or is it just damage? / bite?

Circle what your talking about maybe. Also about what size is the fish? If acting chargy/scraping on things it could be ich. Hard to tell from the photos but I also realize how hard it is to take the photos. Maybe an expert will chime in. There's also a better place to post this in the forums.
 
Fish disease treatment and diagnosis is the forum on here i was referring to. Is the fish acting irritated?
 
I doubt its anything serious. One thing I'm learning more and more is proper nutrition for my fish. There could be some diet relation but thats my only other thought should be all good. Just post again if it gets worse.
 
It may be flukes. My royal gramma periodically scratched his gills. If it is that, what worked for me was to treat the fish in Hydroplex in a 10 minute saltwater dip, then treat in hospital tank with copper or Seachem Cupramine. You will have to treat all your fish and let your tank go fallow.
