Perc infected with something gnarly

Had this guy for 6 years and something is infecting him eating his flesh and I feel really bad and want to help him. What is it and what can I do?

My only guess is some sort of fin rot? What's the best way to treat whatever it is? I have a hospital tank if need by but I'd rather not move him if possible. He had this before about a year ago on his other side and it just went away. but this time it's really bad. He hasn't eaten in days and just lays in this toadstool panting heavily.

All my parameters are fine and I just did a 20%WC last night.

any help is greatly appreciated, feel so bad for the little guy and want to help...
