Permanent white spot

Has anybody else had what appears to be a permanent white spot on a fish that is not ich related? I have a Pakastani angel that has a well defined white spot on his tail fin for 3 months. I thought it might be Ich so I went with hypo-salinity for 30 days. No other fish had any spots including the angel. 30 days later the same spot in the exact same place. Any thoughts? Anybody else see that?
 
Tried many pics with good Nikon camera, but he is always moving fast, especially if he sees me. gets excited about food I guess. But I did get two good pics of his tail, and for some reason or other on the computer it doesn't show up. Tail fin looks perfect, even though I can see the spot from 5 feet away. Go figure...

Any spot that stays in the same location for longer than 72 hours isn't ich. Indeed, even flukes won't hang around in one spot like that. There are three possibilities: Lymphocystis (common on this species, non-fatal, and usually goes away by 3 months). A broken fin ray that has healed with a white spot, may be permanent. A xenoma cyst from a microsporidian...I've never seen just one on a fish, and never on that species.
