Plate coral help

Needing some help with this pinwheel/plate coral.

Ive had it just about 3 weeks now, for the first week or so it was doing “ok” but it’s pretty steadily going downhill.

I’ve had it in multiple places in the tank. Currently in a lower/med flow area with about 75-100 par. Tried feeding Reefroids a few times aswell.

Im having the same issues with a couple Monti frags my buddy gave me. I think your PO4 and Nitrates could be the problem, I just introduced some Chaeto in a refugium and one of the monti's is starting to extend it's polyps. I'd look at a means to reduce both of those.

My parameters were about the same as yours.
 
Any luck? I just tried a new means to fix my issue and I think its working. I think the monti's werent liking my white lights, currently have blues on at about 30% from 1000-2200 and my whites are at 5% from 1400-2000. My plating variety is getting really good with its polyp extension right now. Plan now is to slowly increase the lighting back to where it was.
