He’s in a 480 gallon display with two eels, a coral banded cat shark, a few triggers and tangs

Nice pilot fish. Anyone take a bite out of him? Looks like a bite or injury that has developed secondary bacterial issues. Pic under white light would make it easier to see.
Treatment will be in a separate quarantine setting using Seachem kanaplex or neoplex

Sorry he’s hard to get a good pic

Is there anything reef safe that can be dosed to the main display? Getting him out of a 96x48 tank will be nearly impossible

What size is it? This could be a rub mark.
I can't tell, it seems really emaciated above its eyes.....

Sorry, this is my least favorite fish. Even the Georgia Aquarium had issues with theirs - they get huge. The sell them as tank raised babies, but nobody has a tank large enough to house them permanently.

Jay,

the tank is a 96”x48x24 480 gallon display. He eats like a pig so I’m not sure why he would be that way.

I would treat the fish with antibiotics. But, I'd be hesitant to do that in a display. I also don't know how sharks handle those.

Big enough, for now. You probably already know, but for those who don't, 3+ feet fish eventually

Maybe internal parasites?

That is probably a rub mark…I’ve had that issue with jacks even when kept in round tanks. All it takes is for this fish to brush that spot as it swims around the tank just once in awhile, then the rub mark starts up. They can also get infected.

Agree with the others - my first impression was 'he is very thin'.
